Orforglipron for Type 2 Diabetes: Foundayo Results, FDA Status
Orforglipron (brand name Foundayo) is FDA-approved for weight loss, but not yet for type 2 diabetes in the US. Lilly filed for the diabetes use in the second quarter of 2026, and the FDA has not announced a decision yet. In the ACHIEVE trials it lowered A1C by up to about 2.2 points and beat oral semaglutide head to head, and a 2,749-person trial found no added heart risk versus insulin. Since weight loss can cost muscle, Sharpy gives you one protein target and one 20-minute workout a day to protect it.
Is orforglipron approved for type 2 diabetes, and how well does it work for blood sugar? Here is the short answer. Orforglipron, sold as Foundayo, is FDA-approved for weight loss but not yet for type 2 diabetes in the US. Eli Lilly filed for the diabetes use in the second quarter of 2026, and the FDA had not announced a decision as of October 19, 2026. In Lilly's ACHIEVE trials, the pill lowered A1C by up to about 2.2 points and helped people lose weight. In the only head-to-head trial of two GLP-1 pills, it beat oral semaglutide, the drug in Rybelsus.
Its heart safety data is back in the news this week. That matters because the FDA looks hard at heart risk for any new diabetes drug. This guide sums up the ACHIEVE program in plain words.
Is orforglipron approved for type 2 diabetes?
Here is where things stand in each place.
| Region | Status | Date |
|---|---|---|
| United States | Approved for weight management as Foundayo | April 1, 2026 |
| United States | Type 2 diabetes application under FDA review | Filed Q2 2026 |
| United Kingdom | Authorized for weight management and type 2 diabetes | August 10, 2026 |
Lilly filed the US diabetes application under the FDA's Commissioner's National Priority Review Voucher program, which aims for a faster review. Lilly confirmed the filing in its second-quarter 2026 results. Lilly reports third-quarter results on October 29, 2026, which may bring an update. For the basics on the pill itself, see our Foundayo (orforglipron) guide.
Can you take Foundayo if you have type 2 diabetes?
Yes, if you qualify for the weight-loss use. The US label covers adults with obesity, or adults with overweight and at least one weight-related health problem. Type 2 diabetes is one of those problems.
The label is also built partly on people with diabetes. One of its two main trials, which the label calls Trial 2, enrolled 1,613 adults with type 2 diabetes and a BMI of 27 or more. After 72 weeks, average weight loss was:
- 9.6% on the 17.2 mg dose
- 7% on 9 mg
- 5.1% on 5.5 mg
- 2.5% on placebo
People with diabetes lost less than people without it (11.1% on the top dose in Trial 1). That is common with every GLP-1 drug.
What changes after a diabetes approval is mostly paperwork. The label would add blood sugar control as an official use. That can make insurance coverage easier through a plan's diabetes benefit. Your prescriber decides what fits your health and your other medicines.
Orforglipron clinical trial results in type 2 diabetes
Lilly ran five phase 3 ACHIEVE trials with more than 6,000 people with type 2 diabetes. Here are the main results, from Lilly's announcements and the published papers.
| Trial | Compared with | Length | A1C drop on orforglipron | Weight loss, top dose |
|---|---|---|---|---|
| ACHIEVE-1 (early diabetes, no other drugs) | Placebo | 40 weeks | 1.3 to 1.6 points | 7.9% |
| ACHIEVE-2 (on metformin) | Dapagliflozin (Farxiga) | 40 weeks | Up to 1.7 points (vs 0.8) | 7.3% (vs 3.0%) |
| ACHIEVE-3 (on metformin) | Oral semaglutide 7 mg and 14 mg | 52 weeks | 1.9 to 2.2 points (vs 1.1 to 1.4) | 9.2% (vs 5.3%) |
| ACHIEVE-4 (higher heart risk) | Insulin glargine | 52 weeks, followed to 104 | 1.6 points (vs 1.0) | 8.8% (vs a 1.7% gain) |
| ACHIEVE-5 (on insulin glargine) | Placebo | 40 weeks | Up to 2.1 points (vs 0.8) | 6.1% (vs a 0.6% gain) |
A few notes so you read this table correctly:
- These are "efficacy estimand" numbers. They show the result if everyone had stayed on the drug, so they look a bit better than real-life averages.
- The trials enrolled different people with different starting A1C levels, so compare drugs within a trial, not across trials.
- In ACHIEVE-3, 37.1% of people on the top dose reached an A1C under 5.7%, the normal range, compared with 12.5% on oral semaglutide 14 mg.
Orforglipron vs oral semaglutide (Rybelsus)
ACHIEVE-3 was the first phase 3 trial to test two GLP-1 pills against each other in type 2 diabetes. It enrolled 1,698 adults on metformin. Orforglipron 9 mg and 17.2 mg beat oral semaglutide 7 mg and 14 mg on A1C and on weight.
The trade-off was side effects. More people stopped orforglipron because of side effects: 8.7% and 9.7% on the two doses, versus 4.5% and 4.9% on oral semaglutide.
The daily routine is also different. Rybelsus must be taken on an empty stomach with a small sip of water, then you wait 30 minutes before eating. Orforglipron can be taken any time, with or without food. For more on the older pill, see our Rybelsus guide.
Orforglipron cardiovascular outcomes trial (ACHIEVE-4)
The FDA wants proof that a new diabetes drug does not raise heart risk. ACHIEVE-4 was that test. It enrolled 2,749 adults in 15 countries who had type 2 diabetes, overweight or obesity, and higher heart risk. Half got orforglipron and half got insulin glargine.
| Result | Hazard ratio (95% CI) | What it means |
|---|---|---|
| Major heart events (MACE-4), primary goal | 0.84 (0.59 to 1.20) | No added heart risk; goal met |
| MACE-3 (heart death, heart attack, stroke) | 0.77 (0.52 to 1.13) | Lower, but not statistically proven |
| Death from any cause | 0.43 (0.25 to 0.75) | Exploratory; needs more study |
The trial was designed to rule out harm, not to prove a heart benefit. The lower death rate looks good, but Lilly notes it was not controlled for multiple testing. Treat it as a promising signal, not a proven benefit. Lilly also reported no liver safety signal in this trial. To see how heart benefits were proven for other GLP-1s, read our GLP-1 and cardiovascular health guide.
Side effects and low blood sugar
Side effects in the diabetes trials matched the GLP-1 class. The most common were nausea, diarrhea, vomiting, indigestion and lower appetite. In ACHIEVE-4, 10.6% of people stopped orforglipron because of side effects. If nausea hits, our guide to managing nausea on GLP-1 medications can help.
Low blood sugar is the one to watch if you have diabetes. In Trial 2 of the label, readings under 54 mg/dL happened in 2% of people on orforglipron versus 0.2% on placebo. Among people who also took a sulfonylurea, it was 7%. The label says the risk is also higher with insulin. Your doctor may need to lower those doses, so do not change them on your own.
The label also warns that eye problems from diabetes (retinopathy) can get worse for a while when blood sugar drops fast. Tell your doctor if you have retinopathy or notice changes in your vision.
Protect your muscle while your A1C drops
Losing weight helps blood sugar, but some of that weight can come from muscle. Muscle is where your body stores and burns a lot of its sugar, so keeping it matters even more with diabetes. Two habits help most:
- Hit a daily protein target. See how much protein you need on a GLP-1.
- Lift 2 to 3 times a week. Our guide to resistance training on GLP-1 shows an easy start.
Sharpy turns both into one daily plan: one protein target, one 20-minute workout and one Shape Score. It does not track blood sugar or replace your diabetes care. It helps make sure the weight you lose is fat, not muscle.
Bottom line
Orforglipron is not FDA-approved for type 2 diabetes yet, but the application is filed and the UK already allows it. In the ACHIEVE trials it lowered A1C by up to about 2.2 points, beat oral semaglutide head to head, and showed no added heart risk versus insulin. People with diabetes can already get Foundayo if they qualify for the weight-loss use. Ask your doctor how it fits with your current medicines. Whatever you take, Sharpy can help you keep your muscle while the scale moves.

Questions people ask
Is orforglipron approved for type 2 diabetes?
Not in the US yet. The FDA approved orforglipron as Foundayo on April 1, 2026 for weight management only. Lilly submitted it to the FDA for type 2 diabetes in the second quarter of 2026, and no decision had been announced as of October 19, 2026. The UK regulator, the MHRA, authorized it for both weight management and type 2 diabetes on August 10, 2026.
Can people with type 2 diabetes take Foundayo now?
Yes, if they qualify for the weight-loss use. The US label covers adults with obesity, or adults with overweight and at least one weight-related condition, and type 2 diabetes counts as one of those conditions. One of the two trials behind the approval enrolled only people with type 2 diabetes. Your prescriber decides if it fits you and how it works with your other diabetes medicines.
How much does orforglipron lower A1C?
In Lilly's phase 3 ACHIEVE trials, A1C fell by about 1.3 to 2.2 points, depending on the trial, the dose and what other medicines people took. In the head-to-head ACHIEVE-3 trial, the highest dose lowered A1C by 2.2 points at 52 weeks, compared with 1.4 points for oral semaglutide 14 mg.
Is orforglipron better than Rybelsus?
In ACHIEVE-3, the only head-to-head phase 3 trial of two GLP-1 pills in type 2 diabetes, orforglipron lowered A1C and weight more than oral semaglutide (the drug in Rybelsus) at 7 mg and 14 mg. Orforglipron also has no food or water rules. It is not yet FDA-approved for diabetes, so Rybelsus is still the only US-approved GLP-1 pill for blood sugar.
Is orforglipron safe for your heart?
In the ACHIEVE-4 trial of 2,749 adults with type 2 diabetes and higher heart risk, orforglipron met its goal of showing no added risk of major heart events compared with insulin glargine (hazard ratio 0.84). The trial was built to rule out harm, not to prove a heart benefit. A lower rate of death from any cause was seen, but that was an exploratory finding.
Does orforglipron cause low blood sugar?
It can, mostly when taken with insulin or a sulfonylurea. In the label's type 2 diabetes weight-loss trial, low blood sugar under 54 mg/dL happened in 2% of people on orforglipron versus 0.2% on placebo, and in 7% of people who also took a sulfonylurea. Your doctor may need to lower those medicines.
