Foundayo (Orforglipron): Side Effects, Results, and Cost

The short answer

Foundayo is the brand name for orforglipron, a once-a-day GLP-1 pill from Eli Lilly. The FDA approved it for weight loss in adults on April 1, 2026, and you can take it at any time of day, with or without food. In the main trial, people on the top dose lost about 11% of their body weight in 72 weeks, and the most common side effects were nausea, constipation, diarrhea and vomiting. Because less food can also mean less muscle, eat enough protein and lift a few times a week; Sharpy gives you one protein target and one 20-minute workout a day to make that easy.

What is Foundayo, does it work, and what are the side effects? Here is the short answer. Foundayo is the brand name for orforglipron, a once-a-day GLP-1 pill from Eli Lilly. The FDA approved it for weight loss in adults on April 1, 2026. In the main trial, people on the highest dose lost about 11% of their body weight over 72 weeks. The most common side effects were stomach problems like nausea and constipation. Unlike other GLP-1 pills, you can take it any time of day, with or without food.

Foundayo is back in the news this week. On October 16, 2026, a key European Medicines Agency committee recommended it for approval in the EU. It was already approved in the US, and the UK approved it in August. That news has more people asking what this pill is and whether it fits them.

This guide covers what the label and the trials actually say, in plain words.

What is orforglipron, and what is its brand name?

Orforglipron is the drug. Foundayo is its brand name in the US. It works on the same GLP-1 receptor as Ozempic, Wegovy and Zepbound. It slows how fast your stomach empties and turns down hunger and "food noise."

There is one big difference. Semaglutide and tirzepatide are peptides, which are small proteins. Orforglipron is a small-molecule drug, closer to a regular pill. That is why it does not need special timing to get absorbed. If you want the basics of how this drug class works, see how GLP-1 receptor agonists work.

Orforglipron FDA approval: who is it for?

The FDA label says Foundayo is for:

  • Adults with obesity, or
  • Adults with overweight who also have at least one weight-related health problem

It is meant to be used with a reduced-calorie diet and more physical activity. It is a prescription drug. Your doctor decides if it is right for you.

Some people should not take it. The label says not to use it if you or a family member has had medullary thyroid carcinoma (a rare thyroid cancer) or if you have a condition called MEN 2. Like other GLP-1 drugs, it carries a boxed warning about thyroid C-cell tumors. Our guide to the GLP-1 thyroid warning explains what that warning means.

How do you take Foundayo?

You take one tablet a day, by mouth, at any time of day. You can take it with or without food. You swallow it whole.

Your doctor starts you on a low dose and raises it slowly. The label lists six strengths: 0.8 mg, 2.5 mg, 5.5 mg, 9 mg, 14.5 mg and 17.2 mg. Each step lasts at least 30 days. Do not change your dose on your own. That decision belongs to your prescriber.

Orforglipron weight loss: how well does it work?

The main trial ran for 72 weeks in adults without diabetes. Here are the numbers from the FDA label.

Group Average weight change Lost 10% or more Lost 15% or more
Placebo -2.1% 13% 6%
Foundayo 5.5 mg -7.4% 32.5% 14.3%
Foundayo 9 mg -8.3% 39.8% 20.1%
Foundayo 17.2 mg -11.1% 54.5% 35.9%

A few things to know:

  • These averages include everyone, even people who stopped taking the pill.
  • Among people who stayed on the top dose, Lilly reports an average loss of 27.3 pounds, or 12.4%.
  • Some people lose much more than the average, and some lose less.

The label also says Foundayo caused more fat loss than lean mass loss. That is good news, but "more fat than lean" still means some lean mass is lost. Lean mass includes muscle.

Foundayo side effects

Most side effects affect your stomach and gut. They show up most when you start or when the dose goes up. Here are the four most common, from the label's pooled trial data.

Side effect Placebo 5.5 mg 9 mg 17.2 mg
Nausea 10% 26% 34% 35%
Constipation 9% 20% 27% 24%
Diarrhea 11% 21% 23% 25%
Vomiting 4% 13% 21% 24%

Other common side effects on the label include indigestion, belly pain, headache, a swollen belly, tiredness, burping, reflux, gas and hair loss.

In the trials, about 8% of people on Foundayo stopped because of side effects, compared with 3% on placebo.

The label also warns about less common but serious problems:

  • Pancreatitis. Call your doctor for severe belly pain that will not go away, with or without vomiting.
  • Gallbladder problems. Watch for pain in the upper right belly, fever or yellow skin.
  • Dehydration and kidney problems, mostly from vomiting or diarrhea.
  • Low blood sugar, mainly in people who also take insulin or a sulfonylurea.
  • Risk during anesthesia. Tell your care team you take it before any surgery or procedure.

Foundayo and birth control pills

This one surprises people. The label says if you take birth control pills, you should switch to a non-oral method or add a barrier method (like condoms) for 30 days after you start Foundayo and for 30 days after each dose increase. Non-oral methods like an IUD, patch or implant are not expected to be affected. If you are pregnant or trying to get pregnant, talk to your doctor before starting. The label says to stop Foundayo when pregnancy is known.

Foundayo vs Wegovy pill

The US now has two GLP-1 pills for weight loss. They have not been tested against each other for weight loss, so be careful comparing numbers from different trials.

Foundayo Wegovy pill
Drug Orforglipron Oral semaglutide 25 mg
Maker Eli Lilly Novo Nordisk
FDA approval April 2026 December 2025
How you take it Any time, with or without food Empty stomach, up to 4 oz of water, wait 30 minutes
Average weight loss in main trial 11.1% at 72 weeks 13.6% at 64 weeks
Self-pay starting price $149/month (lowest dose) Check NovoCare

The biggest real-life difference is the timing rule. With the Wegovy pill (and Rybelsus), coffee or food too soon means less drug gets absorbed. Foundayo has no such rule. For more on the older semaglutide pill, read our Rybelsus guide. For the injections, see our GLP-1 medication comparison.

How to protect your muscle on Foundayo

Any GLP-1 drug makes you eat less. When you eat less, your body can burn muscle along with fat, unless you give it a reason to keep the muscle. Two habits matter most:

  1. Eat enough protein every day. Protein at each meal gives your muscles what they need to stay. Our guide on how much protein you need on a GLP-1 shows how to find your number.
  2. Lift something 2 to 3 times a week. Strength training tells your body to keep its muscle. See resistance training on GLP-1 for an easy start.

This is the part a pill cannot do for you. Sharpy turns it into a simple daily plan. You get one protein target sized for you, one 20-minute workout, and one Shape Score that shows whether today helped you lose fat, not muscle.

Foundayo cost and how to get it

Here is what Lilly has announced:

  • Self-pay: starting at $149 a month for the lowest dose through LillyDirect. Higher doses may cost more.
  • Commercial insurance: as little as $25 a month with the savings card, if your plan covers it. Terms apply.
  • Medicare: eligible people may pay $50 a month through the Medicare GLP-1 Bridge program, which began July 1, 2026. It requires prior authorization and has BMI rules.

Prices and programs change often. Check LillyDirect or your insurer for today's price. Our GLP-1 cost and coverage guide explains how to work with insurance.

The bottom line

Foundayo gives people a GLP-1 option with no needle and no timing rules. It works for many people, but its average weight loss in trials was a bit lower than the leading injections. Side effects are mostly in the gut and often ease with time. The decision to start, change or stop belongs to you and your prescriber.

Whatever drug you take, the goal is to lose fat and keep your muscle. That comes down to protein and strength work, every day. Sharpy keeps both in one place on your iPhone, so you can see each day if you are on track.

Questions people ask

What is Foundayo?

Foundayo is the brand name for orforglipron, a GLP-1 pill made by Eli Lilly. It is taken once a day by mouth. The FDA approved it on April 1, 2026 for adults with obesity, or adults with overweight who also have a weight-related health problem, along with a reduced-calorie diet and more activity.

What are the side effects of orforglipron?

The most common side effects on the label are nausea, constipation, diarrhea, vomiting, indigestion, belly pain, headache, a swollen belly, tiredness, burping, reflux, gas and hair loss. Most are stomach-related and tend to show up when the dose goes up. Call your doctor for severe belly pain, signs of an allergic reaction, or vomiting you cannot stop.

How much weight can you lose on Foundayo?

In the 72-week trial of adults without diabetes, people on the highest dose (17.2 mg) lost 11.1% of their body weight on average, compared with 2.1% on placebo. Among people who stayed on the top dose, Lilly reports an average of 27.3 pounds (12.4%). Results vary a lot from person to person.

Is Foundayo better than the Wegovy pill?

They have not been compared head to head for weight loss. In separate trials, the Wegovy pill (oral semaglutide 25 mg) showed about 13.6% average weight loss at 64 weeks, and Foundayo showed 11.1% at 72 weeks. Foundayo is easier to take because it has no food or water rules. Which one fits you is a question for your prescriber.

Lilly lists self-pay pricing starting at $149 a month for the lowest dose through LillyDirect. With commercial insurance that covers it, the savings card can bring it as low as $25 a month. Eligible Medicare patients may pay $50 a month through the Medicare GLP-1 Bridge program, which began July 1, 2026.

Can you take Foundayo with food?

Yes. The label says to take Foundayo once a day, at any time, with or without food. Swallow the tablet whole and take only one tablet per day. This is different from the Wegovy pill and Rybelsus, which must be taken on an empty stomach with a small sip of water.

Does Foundayo affect birth control pills?

It can. The label says people taking oral hormonal birth control should switch to a non-oral method, or add a barrier method, for 30 days after starting Foundayo and for 30 days after each dose increase. Ask your doctor what is right for you.
