GLP-1 Pill vs Shot: Which Works Better for Weight Loss?
For weight loss, the GLP-1 shots still work a bit better than the pills in trials: the Zepbound shot averaged about 21% loss and the Wegovy 2.4 mg shot about 15%, while the Wegovy pill averaged 13.6% and the Foundayo pill about 11%. No trial has tested a pill against a shot head to head, so these are separate studies. Side effects are similar, mostly nausea and other stomach problems. The pill trades a weekly needle for a daily dose, and self-pay prices start lower ($149 a month). Whichever you take, guard your muscle with Sharpy: one protein target and one 20-minute workout a day.
Is a GLP-1 pill as good as a shot? In trials, the shots still lead on weight loss. The Zepbound shot averaged about 21% and the Wegovy 2.4 mg shot about 15%, while the Wegovy pill averaged 13.6% and the Foundayo pill about 11%. But the gap between the Wegovy pill and the Wegovy shot is small. Side effects are about the same. The real differences are how you take it, how often, and what you pay.
One caution before the numbers. No trial has put a GLP-1 pill and a GLP-1 shot against each other for weight loss. Every figure below comes from its own trial, with different people and different lengths. Use them as a rough guide, not a ranking.
GLP-1 pill vs shot: the quick comparison
There are two GLP-1 pills approved for weight loss in the US today. The Wegovy pill (oral semaglutide, Novo Nordisk) was approved in December 2025. Foundayo (orforglipron, Eli Lilly) was approved on April 1, 2026. The main shots are the Wegovy pen (semaglutide) and Zepbound (tirzepatide).
| Wegovy pill | Foundayo pill | Wegovy shot | Zepbound shot | |
|---|---|---|---|---|
| Drug | Semaglutide | Orforglipron | Semaglutide | Tirzepatide |
| How often | Once a day | Once a day | Once a week | Once a week |
| Top or usual dose | 25 mg | 17.2 mg | 2.4 mg (7.2 mg also approved) | 15 mg |
| Average weight loss in main trial | 13.6% (64 weeks) | about 11% (72 weeks) | 14.9% (68 weeks) | 20.9% (72 weeks) |
| Placebo in that trial | 2.4% | 2.1% | 2.4% | 3.1% |
| Food rules | Empty stomach, 30-minute wait | None | None | None |
| Needle | No | No | Yes | Yes |
| Self-pay price (Oct 2026) | $149 to $299 a month | $149 to $299 a month | $349 a month (2.4 mg) | $299 to $449 a month |
These numbers count everyone who started the trial, including people who stopped. People who kept taking the drug as directed lost more: Novo Nordisk reported 16.6% for the Wegovy pill, and Lilly reported 12.4% for the top Foundayo dose.
Does the GLP-1 pill or shot work better?
For pure weight loss, the shots come out ahead, and Zepbound by a wide margin. Zepbound works on two hormone targets (GLP-1 and GIP), not just one, which helps explain why it leads.
The more useful comparison is the same drug in two forms. The Wegovy pill and the Wegovy shot are both semaglutide. The 25 mg pill is a much bigger number than the 2.4 mg shot because the stomach absorbs very little of a pill. The label says the 25 mg tablet gives blood levels close to the 2.4 mg weekly shot. That is why the results land close together: 13.6% for the pill and 14.9% for the shot.
If you need the most weight loss, the strongest options are still shots. Besides Zepbound, the FDA approved a higher 7.2 mg Wegovy dose (Wegovy HD) in March 2026, which averaged about 19% in its 72-week trial. It is for people who already handle the 2.4 mg dose.
GLP-1 pill vs shot side effects
The side effects are mostly the same, because the drugs work the same way. The most common for both are:
- Nausea
- Diarrhea
- Vomiting
- Constipation
- Stomach pain
They are usually worst when you start or move up a dose. In the Wegovy pill trial, 6.9% of people on the pill stopped because of side effects, versus 5.9% on placebo. The serious warnings on the labels, such as pancreatitis, gallbladder problems and a boxed warning about thyroid C-cell tumors in animal studies, apply to semaglutide in both forms. Foundayo has its own label with similar stomach effects.
Two real differences: shots can cause bumps, redness or itching where you inject (see our injection site reactions guide). Pills cannot. On the other hand, a daily pill means 365 doses a year to remember instead of 52.
Pros and cons of a GLP-1 pill vs a shot
Pill pros
- No needles, which matters a lot to some people.
- Lower self-pay starting price ($149 a month for the lowest dose of either pill).
- No pens to keep in the fridge or pack for travel.
- Foundayo can be taken any time of day, with or without food.
Pill cons
- Slightly less weight loss than the shots in trials, and much less than Zepbound.
- A dose every day. Missing days is easier than missing a weekly shot.
- The Wegovy pill has strict rules: take it first thing in the morning on an empty stomach with no more than 4 ounces of plain water, swallow it whole, and wait at least 30 minutes before you eat, drink anything else or take other pills. Our Wegovy pill dosage chart walks through the routine.
Shot pros
- The most weight loss available today, especially with Zepbound.
- Once a week, with no food timing.
- Longer track record, including heart and other outcome data for semaglutide.
Shot cons
- Needles and possible injection site reactions.
- Higher cash prices at maintenance doses.
Weight loss on either one can cost you muscle
Pill or shot, a GLP-1 cuts your appetite. When you eat much less, part of the weight you lose can be muscle, not fat, unless you eat enough protein and use your muscles. This is the same problem on every GLP-1, and the form of the drug does not change it.
Two habits help most: a daily protein target sized to your body (our protein target guide shows the math) and strength work a few times a week. Sharpy puts both in one place on your iPhone: one protein target, one 20-minute workout and a daily Shape Score, so you can see that you are losing fat, not muscle.
GLP-1 pill vs shot cost
Here is what self-pay looked like in our checks of NovoCare, LillyDirect and TrumpRx from October 18 to 22, 2026. Prices change often, so check before you fill.
| Medicine | Self-pay per month |
|---|---|
| Wegovy pill | $149 (1.5 mg), $199 (4 mg), $299 (9 mg and 25 mg) |
| Foundayo pill | $149 to $299, by dose |
| Wegovy pen | $349 (up to 2.4 mg), $399 (7.2 mg), $199 for a new patient's first two fills at the lowest doses |
| Zepbound | $299 (2.5 mg) to $449 (7.5 to 15 mg with on-time refills) |
With commercial insurance that covers weight loss, savings cards can bring any of them down to as little as $25 a month. Eligible Medicare patients may pay $50 a month through the Medicare GLP-1 Bridge. Our weight loss pill cost guide breaks the pill prices down further.
Can you switch from a GLP-1 shot to a pill?
Yes, some people do, and there is now trial data on it. In Lilly's ATTAIN-MAINTAIN trial, published in Nature Medicine in May 2026, adults who had lost weight on the Wegovy or Zepbound shot switched to daily Foundayo for a year. Those coming from Wegovy kept their weight within about 0.9 kg (2 pounds) on average. Those coming from Zepbound regained about 5 kg (11 pounds) on average, but less than people switched to placebo.
The Wegovy pill label also gives steps for moving from the Wegovy shot to the pill. A switch is a prescriber decision. Do not stop a shot or start a pill on your own.
Who might pick which?
This is a talk for you and your prescriber, but these are the questions that usually decide it:
- Do you hate needles? A pill removes that barrier.
- Is maximum weight loss the goal? The trial data favor Zepbound or a higher-dose Wegovy shot.
- Can you keep a strict morning routine? If not, Foundayo or a weekly shot may fit better than the Wegovy pill.
- What does your plan cover? Coverage often decides more than anything else.
- Do you have type 2 diabetes? Rybelsus is a semaglutide pill for diabetes only. See our Rybelsus guide and the full GLP-1 medication comparison.
If you are pregnant, planning a pregnancy, under 18 or have a history of an eating disorder, talk to your doctor before starting any GLP-1.
The bottom line
GLP-1 shots still give the most weight loss, and Zepbound leads the pack. The Wegovy pill comes close to the Wegovy shot, and Foundayo trades a little weight loss for the easiest routine. Side effects are about the same. Pick with your prescriber based on results, routine, needles and cost. Then protect the muscle you have: Sharpy gives you one protein target, one 20-minute workout and one Shape Score a day on whatever GLP-1 you take.

Questions people ask
Is the GLP-1 pill as effective as the shot?
Close, but not quite, based on separate trials. The Wegovy pill (25 mg) averaged 13.6% weight loss at 64 weeks and Foundayo (17.2 mg) about 11% at 72 weeks. The Wegovy shot (2.4 mg) averaged 14.9% at 68 weeks and the Zepbound shot (15 mg) 20.9% at 72 weeks. No head-to-head trial of a pill versus a shot for weight loss has been published.
Does the GLP-1 pill have fewer side effects than the shot?
Not really. The labels list the same main side effects for pills and shots: nausea, diarrhea, vomiting and constipation. In the Wegovy pill trial, 6.9% stopped because of side effects versus 5.9% on placebo, similar to the shot. The pill avoids injection site reactions, which is one real difference.
Which is cheaper, the GLP-1 pill or the shot?
On self-pay, the pills usually cost less. As of our October 2026 checks, the Wegovy pill was $149 to $299 a month and Foundayo $149 to $299 by dose, while the Wegovy pen was $349 a month (2.4 mg) and Zepbound $299 to $449. With insurance, both can cost as little as $25 a month if your plan covers them.
Can I switch from a GLP-1 shot to a pill?
Some people do, but only with your prescriber. In Lilly's ATTAIN-MAINTAIN trial, people who switched from the Wegovy shot to Foundayo kept their weight within about 0.9 kg (2 pounds) after a year, and people who switched from Zepbound regained about 5 kg (11 pounds) on average. The Wegovy pill label also explains how to switch from the Wegovy shot.
Which GLP-1 pill is best for weight loss?
In separate trials, the Wegovy pill averaged a bit more weight loss (13.6%) than Foundayo (about 11%). Foundayo is easier to take because it has no food or water rules. Rybelsus is also a semaglutide pill, but it is approved only for type 2 diabetes, not weight loss. Your prescriber can help you weigh results against convenience.
Why do GLP-1 pills have to be taken every day?
Pills leave the body faster than the weekly shots, which are built to last about a week. A daily dose keeps the drug level steady. The Wegovy pill also needs a strict routine: an empty stomach, up to 4 ounces of plain water, and a 30-minute wait before food, drinks or other pills.
