Revita Procedure (Fractyl): Keeping Weight Off After GLP-1
Revita is a one-time endoscopic procedure from Fractyl Health that resurfaces the lining of the duodenum, the first part of the small intestine. In the pivotal REMAIN-1 study, people who stopped tirzepatide and then had Revita regained 5.1% of body weight at six months, versus 8.2% after a sham procedure. It is not FDA-approved: Fractyl plans to file in late 2026 and hopes for a US launch in early 2028. Until then, the proven tools for keeping weight off are protein, strength training and tracking, which is what Sharpy is built for.
What is the Revita procedure, and can it really keep the weight off after you stop a GLP-1? Revita is a one-time endoscopic procedure from Fractyl Health that treats the lining of the duodenum, the first part of the small intestine. In its pivotal REMAIN-1 study, people who stopped tirzepatide and had Revita regained 5.1% of body weight at six months, compared with 8.2% after a sham procedure. It is not FDA-approved yet. Fractyl plans to file with the FDA in late 2026 and hopes for a US launch in early 2028.
Revita was in the news twice this week. On October 16, 2026, Fractyl announced the six-month pivotal results. On October 22, it said it would cut about 45% of its workforce and put all its focus on Revita. Here is what the results mean for someone on Zepbound, Wegovy or Ozempic who worries about life after the shots.
Why weight regain after stopping GLP-1 is the problem Revita targets
Stopping a GLP-1 usually brings weight back. In the SURMOUNT-4 trial, people who switched from tirzepatide to placebo regained about 14% of their body weight within a year. In the STEP 1 extension, people regained about two-thirds of the weight they had lost on semaglutide one year after stopping. We cover the details in our GLP-1 rebound research guide.
That is why "post-GLP-1 weight maintenance" has become its own market. Many people stop because of cost, side effects or insurance changes. Revita is one of the first products built just for that moment.
How the Revita procedure works
| Question | Answer |
|---|---|
| Maker | Fractyl Health (Nasdaq: GUTS), Burlington, Massachusetts |
| What it is | Revita DMR System: duodenal mucosal resurfacing |
| How it is done | A scope goes through the mouth into the duodenum; the inner lining is treated so new lining grows back |
| How often | Once |
| Cuts or implants | None |
| Status in the US | Investigational only; FDA Breakthrough Device Designation |
| Status abroad | CE marked in the EU and UK |
Fractyl's idea is that years of excess weight and high blood sugar change the lining of the duodenum, which helps sense food and sends hormone signals. Resurfacing it is meant to restore healthier signaling. That theory is still being tested.
Fractyl Revita REMAIN-1 results, in plain English
REMAIN-1 is a randomized, double-blind, sham-controlled study at more than 30 US sites. Here is how the pivotal cohort worked:
- Adults with obesity (no type 2 diabetes) took tirzepatide, the drug in Zepbound and Mounjaro, for an average of 25 weeks and lost an average of 18.3% of body weight.
- They stopped tirzepatide.
- They were randomly assigned, two to one, to Revita or a sham procedure. Neither the patients nor the people measuring them knew which one they got.
| Six-month result (company-reported) | Revita | Sham |
|---|---|---|
| Weight regained (% of body weight) | 5.1% | 8.2% |
| Share of lost weight kept off | About 80% | About 67% |
| Weight stable or still dropping (post hoc) | 32% | 18% |
The main difference was statistically significant (p=0.0023, 308 people in the analysis). Fractyl says the gap between the groups nearly doubled from month 3 to month 6.
Fractyl also points to people who got a longer treatment (20 cm or more of the duodenum), who regained 3.8%. That was a post hoc analysis, meaning it was not the main planned test, so treat it as a clue, not a result.
Three things to keep in mind:
- Revita slowed regain. It did not stop it. Both groups gained weight back.
- These are company numbers. The data are not yet peer reviewed or checked by the FDA.
- Six months is short. The second main goal, whether more than half of patients keep at least 5% weight loss at one year, reports in Q1 2027. Investors were not impressed: Fractyl's stock fell more than 60% on the day of the release.
Revita side effects and safety
Fractyl reports no device- or procedure-related serious adverse events, no deaths and no unexpected device effects through the September 24, 2026 data cut-off. Side effects linked to the procedure were mostly mild and happened right around it, with about 94% within seven days. The company has not published a full side effect list yet.
Any endoscopy has risks, and sedation is usually used. Talk to a gastroenterologist about what applies to you. If you are pregnant, under 18 or have a history of an eating disorder, any weight procedure needs a doctor's close review.
Revita FDA approval timeline and cost
| Milestone | Timing (per Fractyl) |
|---|---|
| Six-month pivotal results | October 16, 2026 |
| FDA De Novo submission | Late Q4 2026 |
| CPT code 1122T takes effect | January 1, 2027 |
| One-year results | Q1 2027 |
| Possible FDA authorization | Late 2027 |
| Possible US launch | Early 2028 |
There is no US price. A billing code does not mean insurance will pay. Until approval, the only legitimate way to get Revita in the US is a clinical trial.
Revita vs other ways to keep weight off after a GLP-1
| Option | Status | What we know |
|---|---|---|
| Stay on the GLP-1 | Approved | Best-proven way to keep weight off in trials |
| Taper the dose | Doctor's decision | Studied less; see our tapering guide |
| Strength training and protein | Available now | Helps keep muscle, which supports your metabolism |
| Revita | Investigational | Less regain than sham at 6 months in one company-reported study |
You do not have to wait until 2028 to protect your results. The habits that matter most are the ones you build while you are still on the drug: enough protein, regular strength work and honest tracking. Sharpy sets one daily protein target, gives you one 20-minute workout and turns your progress into a Shape Score, so you can see whether you are keeping muscle, not just losing pounds. Our guide on how to stop a GLP-1 without rebound walks through the plan step by step, and resistance training on GLP-1 shows where to start.
What Revita means for you right now
- If you are on a GLP-1 and doing well, nothing changes. Do not stop or change your dose because of this news. That is a talk with your prescriber.
- If you are about to stop, ask your doctor about a maintenance plan and whether a Revita trial is open near you.
- If you see Revita sold for cash in the US outside a trial, walk away.
Revita is a promising idea with real but modest early data. The one-year results in early 2027 will tell us much more. For now, keep the muscle you have: Sharpy helps you hit protein and train every day, on the drug and after it.

Questions people ask
What is the Revita procedure?
Revita is an endoscopic procedure made by Fractyl Health. A doctor passes a thin scope through the mouth into the duodenum, the first part of the small intestine, and treats its inner lining so a new lining grows back. Fractyl calls it duodenal mucosal resurfacing, or DMR. The goal is to reset how the gut senses food and sends signals about blood sugar and fullness. It is done once, with no cuts on the belly.
Is Revita FDA approved?
No. In the US, Revita is for investigational use only. It has FDA Breakthrough Device Designation for weight maintenance in people with obesity who stop GLP-1 drugs, which speeds up review but is not an approval. Fractyl plans a De Novo submission in late Q4 2026 and says FDA authorization could come in late 2027. It is CE marked in the European Union and the United Kingdom.
How well does Revita work after stopping Zepbound?
In the REMAIN-1 pivotal cohort, adults lost an average of 18.3% of body weight on tirzepatide, then stopped it and got Revita or a sham procedure. At six months, the Revita group had regained 5.1% of body weight and the sham group 8.2%, per Fractyl. That means Revita patients kept about 80% of their lost weight and sham patients about 67%. These are company-reported results, not yet peer reviewed.
What are the side effects of Revita?
Fractyl reports no device- or procedure-related serious adverse events and no deaths in REMAIN-1 through a September 24, 2026 data cut-off. Related side effects were mostly mild and happened around the procedure, about 94% within seven days. The company has not yet published a full list. Full safety data will come with the FDA review.
How much does the Revita procedure cost?
There is no US price yet because Revita is not approved. A Category III CPT billing code for the procedure (1122T) takes effect on January 1, 2027, which is a tracking code, not a promise of insurance coverage. Be wary of anyone offering Revita for cash in the US outside a clinical trial.
Can I get Revita now?
Only in a clinical trial in the US. Fractyl says Revita is CE marked in Europe and the UK, but availability there is limited. If you want to try it, search ClinicalTrials.gov for "Revita" or "REMAIN" and ask your doctor whether you might qualify.
Does Revita replace a GLP-1?
No. Revita is being tested for people who have already lost weight on a GLP-1 and then stop. It did not stop all regain in the trial. Whether to stay on, taper or stop a GLP-1 is a decision for you and your prescriber.
