Insurance Dropping GLP-1 Coverage in 2027? What to Do Now
More US employers are dropping GLP-1 coverage for weight loss in 2027: a Business Group on Health survey of 127 large employers found about 14% have dropped it or plan to, and a Mercer survey found 6% of employers with 500+ workers dropped it in 2026. Cost is the reason, and most plans keep covering GLP-1s for type 2 diabetes. If your plan is dropping Wegovy or Zepbound, read your 2027 plan documents during open enrollment, ask your prescriber whether another covered use applies, and compare self-pay prices, which start at $149 to $299 a month. Whatever you decide, protect your muscle with Sharpy, one protein target and one 20-minute workout a day.
Is your health insurance dropping GLP-1 coverage, and what can you do about it? Yes, more employers are cutting coverage of GLP-1 drugs for weight loss in 2027, mostly because of cost. Most still cover them for type 2 diabetes. If your plan is dropping Wegovy, Zepbound or Foundayo, check your 2027 plan documents now, ask your prescriber about other covered uses, and compare self-pay prices, which start at $149 to $299 a month.
This is open enrollment season. Many people are opening their 2027 benefits packets this month and finding a line that says weight-loss drugs are no longer covered. Below is what the surveys show, how to read your own plan, and the options that cost the least.
Are insurance companies dropping GLP-1 coverage?
Some are, and the number is growing. Here is what the three biggest 2026 employer surveys found.
| Survey | Who was asked | What it found |
|---|---|---|
| Business Group on Health, 2027 strategy survey (August 2026) | 127 large employers, 11 million people covered | About 14% have dropped weight-loss GLP-1 coverage or plan to in 2027. Coverage fell from 72% of employers in 2025 to 60% in 2026. |
| Business Group on Health, GLP-1 survey (May 2026) | 105 large employers | 67% covered GLP-1s for weight loss. Of those, 72% said they would likely keep it in 2027 and 10% said they would likely drop it. |
| Mercer, health and benefit strategies for 2027 (June 2026) | Employers with 500+ workers | 44% cover GLP-1s for obesity. 6% dropped coverage in 2026, 5% plan to or are considering it for 2027, and 27% tightened the rules. |
Two things stand out. First, few plans are adding coverage. A Business Group on Health vice president put it bluntly: "Not a single employer will be adding GLP-1 coverage for weight management" in 2027. Second, many plans are not dropping GLP-1s outright. They are making them harder to get.
Some big names have already moved. Cigna stopped covering weight-loss GLP-1s for its own employees in July 2026, according to Reuters.
Why are insurance companies dropping GLP-1 drugs?
It comes down to money, even though prices are lower than they were a year ago.
- More people are using them. Pills like the Wegovy pill (launched January 2026) and Foundayo (April 2026) brought in people who never wanted a shot. Two-thirds of employers in the August survey reported rising use.
- People stay on them for years. Obesity is treated as a long-term condition, so the cost does not end after a few months.
- Pharmacy bills are climbing. Employers in the August survey said pharmacy is about 25% of their health spending and expected it to rise 12% in 2027.
- Employers say they do not get the cash prices. Reuters reported that employers are not seeing the same discounts through their pharmacy benefit managers that cash buyers get from drug makers.
Insurance dropping GLP-1 coverage vs adding new rules
Read your plan closely, because "not covered" and "harder to get" are very different problems.
| What your plan did | What it looks like | What to do |
|---|---|---|
| Excluded weight-loss drugs | "Weight loss medications are not covered" in the 2027 summary | Ask about other covered uses, other plans, or self-pay |
| Added prior authorization or new criteria | You need a form, a BMI record, or a diagnosis on file | Your prescriber sends the paperwork |
| Requires a weight program | You must enroll in a lifestyle or coaching program to stay covered | Enroll early so there is no gap |
| Limits prescribers | Only certain doctors or virtual programs can prescribe | Check if your prescriber is on the list |
| Switched the preferred drug | One GLP-1 is covered, another is not | Ask your prescriber if the preferred drug fits you |
Will my insurance still cover GLP-1s for diabetes?
Most likely. The cuts are almost all to weight-loss coverage. Ozempic, Mounjaro and other GLP-1s for type 2 diabetes are usually still on the drug list, because uncontrolled diabetes costs plans more.
Some weight-loss drugs also have FDA-approved uses beyond weight loss, and a plan may treat those differently:
| Drug | Other FDA-approved use | Approved |
|---|---|---|
| Wegovy (semaglutide) | Lowers the risk of heart attack, stroke and heart death in adults with heart disease and obesity or overweight | March 2024 |
| Wegovy (semaglutide) | Liver disease called MASH with moderate to advanced scarring | August 2025 |
| Zepbound (tirzepatide) | Moderate to severe obstructive sleep apnea in adults with obesity | December 2024 |
Whether you qualify is your prescriber's call, not yours, and whether your plan pays for that use depends on the plan. Our Zepbound sleep apnea guide explains that approval in detail.
My insurance dropped Zepbound: what do I do?
The same steps work for Wegovy, Foundayo or any other GLP-1.
- Find the reason in writing. Look for the 2027 Summary of Benefits and Coverage and the drug list. If you got a denial letter, it must say why.
- Check every plan your employer offers. Some employers drop GLP-1s from one plan but not another. Open enrollment is your one chance a year to switch.
- Talk to your prescriber before your last covered fill. Ask whether another covered use or a preferred drug fits you. Do not stop, stretch or split doses on your own.
- Appeal when the denial is about rules, not exclusion. If you were turned down for missing paperwork, an appeal can work. Under HealthCare.gov rules you have 180 days to file an internal appeal, and the plan has 30 days to decide on a drug you have not received yet. Our Zepbound appeal letter guide has a template.
- Price out self-pay (next section).
- On Medicare or Medicaid? Different rules apply. See the Medicare GLP-1 Bridge guide and the Medicaid GLP-1 coverage guide.
While you sort this out, keep the habits that protect your result. Sharpy sets one daily protein target and one 20-minute strength workout, so the weight you have already lost keeps coming from fat, not muscle.
What does a GLP-1 cost without insurance?
These are the drug makers' own self-pay prices, checked on TrumpRx.gov, NovoCare and LillyDirect on October 19 to 21, 2026.
| Drug | Self-pay price per month | Per year at that price |
|---|---|---|
| Wegovy pill | $149 (1.5 mg), $199 (4 mg), $299 (9 and 25 mg) | $1,788 to $3,588 |
| Foundayo pill | $149 to $299 by dose | $1,788 to $3,588 |
| Wegovy pen, up to 2.4 mg | $349 ($199 for a new patient's first two fills at the lowest doses) | about $4,188 |
| Zepbound KwikPen | $299 (2.5 mg), $399 (5 mg), $449 (7.5 to 15 mg with on-time refills) | $3,588 to $5,388 |
A few catches. Cash payments usually do not count toward your deductible. Prices can change, and the $199 Wegovy intro price applies to fills made by December 31, 2026. A prescribed GLP-1 paid in cash generally counts as a medical expense for an HSA or FSA under IRS Publication 502. Our TrumpRx GLP-1 guide and weight loss pill cost guide break these prices down by dose.
Skip compounded or "research" GLP-1s as a money saver. FDA does not review them for safety or quality, and several were recalled this fall.
What happens if you lose coverage and stop?
This is the part to plan for. In the STEP 1 extension, people who stopped semaglutide got back about two-thirds of the weight they had lost within one year. Appetite returns, and muscle lost along the way makes regain easier.
Any change to your medicine is a decision for you and your prescriber. What you can do on your own is build the habits that help either way: enough protein at every meal, two or three short strength sessions a week, and a simple way to track your trend. Our guide on how to stop GLP-1 without regaining the weight covers what helps most.
The bottom line
Insurance dropping GLP-1 coverage is real, but it is a growing minority of plans, it mostly hits weight-loss coverage, and many plans are adding rules rather than cutting the drug. Read your 2027 plan now, talk to your prescriber before your last covered fill, and compare self-pay prices. Then protect what you have built with Sharpy: one protein target, one 20-minute workout and one Shape Score a day.

Questions people ask
Why are insurance companies dropping GLP-1 drugs?
Cost. Employers pay for most of these plans, and GLP-1 use keeps growing even as prices fall. In a Business Group on Health survey released in August 2026, 95% of large employers said they were worried about pharmacy costs, and pharmacy spending was expected to rise 12% in 2027. Mercer reported that 27% of large employers tightened GLP-1 rules in 2026 or plan to in 2027.
How many employers are dropping GLP-1 coverage in 2027?
It is a growing minority, not everyone. Business Group on Health found that about 14% of its large-employer members have dropped weight-loss GLP-1 coverage or plan to in 2027, and that coverage fell from 72% of employers in 2025 to 60% in 2026. Mercer found 6% of employers with 500 or more workers dropped coverage in 2026 and another 5% plan to or are considering it for 2027.
Will my insurance still cover Ozempic or Mounjaro for diabetes?
Usually, yes. Most of the cuts are to coverage for weight loss. Employers that dropped weight-loss coverage have generally kept GLP-1s for type 2 diabetes. Check your plan's 2027 drug list (formulary) to be sure, because each plan is different.
My insurance dropped Zepbound. What do I do?
First, find out why: a plan exclusion, a new prior authorization rule, or a switch to a preferred drug are all handled differently. Then talk to your prescriber about whether another covered use applies, such as Zepbound for moderate to severe obstructive sleep apnea, whether a preferred drug on your formulary fits, or whether a self-pay price works for you. Do not stop or change your dose without your prescriber.
Can I appeal if my plan stops covering GLP-1s for weight loss?
You can always ask, but an appeal works best when the plan denied you under its own rules, for example a missing BMI or diagnosis. If the plan simply excludes weight-loss drugs for everyone, an appeal is less likely to succeed. Under HealthCare.gov rules you have 180 days from a denial notice to file an internal appeal.
What happens if I stop my GLP-1 because I lose coverage?
Weight often comes back. In the STEP 1 extension, people who stopped semaglutide regained about two-thirds of the weight they had lost within a year. That is why the decision belongs with your prescriber, and why keeping muscle and protein habits matters before any change.
Can I use my HSA or FSA to pay for a GLP-1 out of pocket?
Prescription drugs are qualified medical expenses under IRS Publication 502, so a prescribed GLP-1 paid in cash generally qualifies. Check your account rules, and keep the prescription and receipts.
