**TL;DR:** In England, Mounjaro NHS criteria in 2026 are a BMI of 35 or more (32.5 for some ethnic groups) plus at least 4 of 5 conditions: type 2 diabetes, high blood pressure, heart disease, sleep apnea and abnormal blood fats. A group with a BMI of 40 or more and 3 conditions is due from April 1, 2027. MPs called the rollout "unambitious" on October 22, 2026, but the rules have not changed yet. While you wait or once you start, [Sharpy](https://apps.apple.com/us/app/glp-diet-app-sharpy/id6762084138) helps you hit a daily protein target and protect muscle.

Can you get Mounjaro on the NHS, and do you meet the criteria? In England, the answer depends on two things: your BMI and how many of five weight-related health conditions you have. Right now, NHS GP services can offer Mounjaro (tirzepatide) for weight loss to adults with a BMI of 35 or more (32.5 for some ethnic groups) who also have at least 4 of these 5 conditions: type 2 diabetes, high blood pressure, heart disease, obstructive sleep apnea and abnormal blood fats. A third group, with a BMI of 40 or more and 3 conditions, is due to be added from April 1, 2027.

This matters this week because MPs on the Commons Health and Social Care Committee called the rollout "unambitious" in a report covered on October 22 and 23, 2026. Here is what the rules say, what the MPs found, and what to do if you do not qualify yet.

## Mounjaro NHS criteria 2026: the cohort table

NICE, the body that decides what the NHS in England pays for, approved tirzepatide for weight management in December 2024 (guidance TA1026). Because so many people qualify, NHS England agreed to roll it out through GP surgeries in stages, called cohorts. The GP rollout started on June 23, 2025.

| Cohort | BMI | Qualifying conditions | Eligible from |
|---|---|---|---|
| Cohort 1 | 40 or more | At least 4 of the 5 | June 23, 2025 |
| Cohort 2 | 35 to 39.9 | At least 4 of the 5 | June 23, 2026 |
| Cohort 3 | 40 or more | At least 3 of the 5 | April 1, 2027 (planned) |

Each new cohort is added on top of the earlier ones. Nobody is removed. The BMI thresholds are lowered by 2.5 points for people from South Asian, Chinese, other Asian, Middle Eastern, Black African or African-Caribbean backgrounds, because health risks start at a lower BMI in these groups. So a cohort 2 threshold of 35 becomes 32.5.

## The 5 conditions that count

The NHS England guidance counts these five:

1. Type 2 diabetes
2. High blood pressure (hypertension)
3. Heart disease (cardiovascular disease)
4. Obstructive sleep apnea
5. Abnormal blood fats (dyslipidemia)

Other conditions, like knee pain or fatty liver, do not count toward the number in the GP rollout, even though they are linked to weight. Your GP practice uses your records to check which ones you have.

## Mounjaro NHS eligibility vs the full NICE criteria

The full NICE recommendation is wider than what GPs can offer today. NICE says tirzepatide can be used for adults with a BMI of at least 35 and at least one weight-related condition, alongside a lower-calorie diet and more activity. The cohort plan narrows that down at first, then widens it over time. NHS England has said about 220,000 people should be treated in the first three years.

That gap is what the MPs objected to. The NHS estimates about 2.8 million people meet the NICE criteria. Treating 220,000 of them over three years is about 6%, a figure the committee took from the Health Foundation. The NHS has said treating everyone eligible in the first year would cost an estimated £15.2 billion over five years, which is why NICE agreed to the slower plan.

## What the MPs' report says about the NHS Mounjaro rollout

The committee, chaired by Layla Moran, made these main points:

- The pace is too slow and is creating a "two-tier system" where access depends on ability to pay. More than 2.37 million packs of GLP-1 medicines are used in the UK each month, and most are bought privately.
- The government should urgently reassess the pace of the rollout, including using the capacity of digital providers.
- Obesity should be managed as a long-term condition, like diabetes, with a national care pathway and local access targets.
- The MHRA, the UK medicines regulator, should be reviewed for its capacity to tackle the illegal trade in weight-loss jabs.

An NHS spokesperson said the service is testing new ways to speed up access, including new routes for starting these medicines. Nothing in the report changes the criteria today. It is a recommendation, not a new rule.

## How to get Mounjaro on the NHS through your GP

If you think you qualify, the steps look like this:

1. **Ask your GP practice.** Eligibility is checked through a triage step, and a trained prescriber does the prescribing. GP practices can choose whether to offer the service, so it is not yet available everywhere. A 2025 BMJ investigation found fewer than half of local NHS bodies had started prescribing a few months in.
2. **Agree to the support program.** You must take part in "wraparound" support on diet, nutrition and physical activity. Without it, the NHS will not prescribe tirzepatide.
3. **Expect a 6-month check.** NICE says that if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your clinician should decide whether to continue.

Standard NHS prescription charges apply in England unless you are exempt. Wegovy and Saxenda are not part of this GP service. If you already buy Mounjaro privately, tell your GP. You can move to an NHS prescription only if you meet the criteria, and you do not jump the queue.

## Mounjaro NHS Scotland, Wales and Northern Ireland

The cohort table above is NHS England's plan. Scotland, Wales and Northern Ireland run their own health services and set their own pathways. In Scotland, each health board decides how and when to offer weight-loss medicines, and some boards have not yet opened referrals. If you live outside England, check your health board's website or ask your GP practice what is available locally.

## Mounjaro NHS cost vs private prices

| Route | What you pay |
|---|---|
| NHS (England) | Standard prescription charge, unless exempt |
| Private pharmacy or online clinic | Set by the seller, on top of Lilly's list price |

Eli Lilly raised its UK list price for private buyers on September 1, 2025. The highest dose went from £122 to £330 a month. NHS prices were not affected. Private sellers set their own prices, so compare carefully and only buy from a registered UK pharmacy. Our [Mounjaro guide](/blog/mounjaro-guide/) explains how the drug works, and the [Mounjaro dosage chart](/blog/mounjaro-dosage-chart/) shows the label's dose steps.

## Mounjaro NHS side effects and safety

The side effects are the same whether you get it from the NHS or privately. The most common are nausea, indigestion, constipation and diarrhea. Rarer but serious risks include gallbladder problems and pancreatitis, and low blood sugar is a risk if you also take insulin or a sulfonylurea. Tirzepatide is not recommended in pregnancy or while breastfeeding, so if you are planning a pregnancy, talk to your clinician about timing. In the UK you can report side effects through the MHRA Yellow Card scheme. Any decision about starting, changing or stopping your dose belongs to your prescriber.

## Protecting muscle while you wait, or once you start

Whether your NHS cohort opens this year or next, the habits that matter are the same. Fast weight loss on tirzepatide can take muscle with it, so most of the work is protein and strength training. A common starting point in research is about 1.2 to 1.6 g of protein per kg of body weight a day, but your own target is something to set with your care team. Our [protein target guide](/blog/protein-target-on-glp1/) shows how to work it out, and the [beginner strength program](/blog/beginner-strength-program-glp1/) needs no gym.

If you want those two habits in one place, the [Sharpy app](https://apps.apple.com/us/app/glp-diet-app-sharpy/id6762084138) gives you one daily protein target, one 20-minute workout and a Shape Score to see whether you are losing fat rather than muscle. It fits well alongside the NHS wraparound support. Sharpy is not part of the NHS and does not prescribe medicine.

## The bottom line

In England, the NHS Mounjaro criteria in October 2026 are a BMI of 35 or more (32.5 for some ethnic groups) plus 4 of the 5 listed conditions, with a 3-condition group due from April 2027. MPs want the rollout sped up, but the rules have not changed yet. Ask your GP practice, be ready to join the support program, and start building the protein and strength habits now. When you begin treatment, [Sharpy](https://apps.apple.com/us/app/glp-diet-app-sharpy/id6762084138) can help you keep the muscle you have. If you ever stop, our guide on [stopping a GLP-1 without rebound](/blog/how-to-stop-glp1-without-rebound/) explains what to plan for.
