**TL;DR:** Every state Medicaid program must cover GLP-1 drugs for type 2 diabetes, and FDA-approved uses like Wegovy for heart risk and Zepbound for sleep apnea, usually with prior authorization. Coverage for weight loss alone is optional, and it is shrinking: KFF counted 13 states in January 2026, down from 16, and Massachusetts ended its coverage in July 2026. A new federal deal called the BALANCE model lets states add weight-loss coverage at a lower price, and Indiana said in July 2026 that it will join. Whatever your coverage, the muscle you keep is up to you, and [Sharpy](https://apps.apple.com/us/app/glp-diet-app-sharpy/id6762084138) turns that into one protein target and one 20-minute workout a day.

Does Medicaid cover GLP-1 drugs in 2026? **Yes for type 2 diabetes everywhere, but for weight loss alone only in some states, and that list got shorter this year.** Every state Medicaid program must cover GLP-1s for their FDA-approved medical uses, like diabetes, lowering heart risk and sleep apnea, usually with prior authorization. Covering them only for obesity is a choice each state makes. KFF counted 13 states doing it in January 2026, down from 16. A new federal deal, the BALANCE model, could bring some states back.

This is in the news again. On October 21, 2026, PennLive looked at what Pennsylvania's cut means for thousands of patients. Pennsylvania was one of four states that dropped weight-loss coverage on January 1. Here is where things stand and what you can do.

## What Medicaid must cover vs what is optional

Federal law splits GLP-1 coverage into two groups. KFF explains that Medicaid must cover nearly all FDA-approved drugs for their approved uses. Drugs used only for weight loss are an exception that states may leave out.

| Use of the GLP-1 | Must your state Medicaid cover it? | Example drugs |
|---|---|---|
| Type 2 diabetes | Yes | Ozempic, Mounjaro, Rybelsus, Trulicity |
| Lowering heart attack and stroke risk (with heart disease and overweight) | Yes | Wegovy |
| Moderate to severe obstructive sleep apnea with obesity | Yes | Zepbound |
| Weight loss or obesity only | No, it is optional | Wegovy, Zepbound, Foundayo, Saxenda |

"Must cover" does not mean "no rules." States can still require prior authorization, a diagnosis code or a preferred brand. If you have one of the conditions in the first three rows, ask your prescriber to submit the request under that condition.

## Which states dropped Medicaid GLP-1 coverage for weight loss?

These changes are confirmed in state notices or KFF's tracking:

| State | What changed | When |
|---|---|---|
| California (Medi-Cal) | Ended GLP-1 coverage for weight loss; diabetes, heart, kidney, MASH and sleep apnea uses still possible | January 1, 2026 |
| Pennsylvania | Ended coverage for overweight and obesity; other uses need prior authorization | January 1, 2026 |
| New Hampshire | Ended coverage for weight loss | January 1, 2026 |
| South Carolina | Ended coverage for obesity | Early 2026 (per KFF) |
| Massachusetts (MassHealth) | Ended coverage of drugs used only for weight loss | July 3, 2026 |
| North Carolina | Dropped coverage in October 2025, then brought it back | Reinstated December 2025 |

California and Pennsylvania both kept a door open for members under 21. Their weight-loss requests can still be reviewed under EPSDT, the federal child health benefit.

The reason was money. KFF found Medicaid spent almost $9 billion on GLP-1s in 2024 before rebates, up from about $1 billion in 2019. In Pennsylvania, the state's own figures put its GLP-1 spending at $1.3 billion for 2025.

State lists change fast, and some trackers online disagree. The only answer that counts is your own plan's. Call the member services number on your card and ask: "Do you cover Wegovy, Zepbound or Foundayo for weight management, and what are the prior authorization rules?"

## The BALANCE model: how Medicaid coverage could come back

In December 2025, CMS launched the BALANCE model. Here is how it works, based on CMS and KFF:

- **CMS bargains for the states.** It negotiates GLP-1 prices with drug makers on behalf of state Medicaid programs. The Medicaid price is confidential.
- **States opt in.** Joining is voluntary. States can start any time from May 2026 through January 1, 2027.
- **Covered drugs:** all forms of Wegovy, Ozempic, Rybelsus, Mounjaro and Foundayo, plus the Zepbound KwikPen.
- **Who qualifies:** members 18 or older whose prescriber attests that they meet clinical criteria. KFF says the obesity criteria are similar to the Medicare GLP-1 Bridge. States can be looser but not stricter.
- **Lifestyle support:** drug makers must offer a free healthy eating and activity program to everyone on the drugs.
- **Your cost:** the model does not change Medicaid copays, which federal law keeps at nominal amounts.

**Indiana is the first state we can confirm.** On July 30, 2026, Governor Mike Braun said Indiana Medicaid will join BALANCE to cover select GLP-1s for eligible members with obesity. The state said drug makers will charge $245 per member per month, and Indiana's share after federal funds is $85.16. Eligibility rules and a start date were still to come. As of late September 2026, the CMS model page did not list participating states.

If you are on Medicare instead, the rules are different. Read our [Medicare GLP-1 Bridge guide](/blog/medicare-glp-1-bridge/) for the $50 copay program.

## Medicaid GLP-1 prior authorization: what to bring

Whether your plan covers the drug for diabetes, heart risk, sleep apnea or obesity, your prescriber will usually file a prior authorization. Make it easy for them:

- Your current height, weight and BMI, plus your weight history
- Diagnoses that may qualify, such as type 2 diabetes, heart disease or a sleep study showing sleep apnea
- Past diets, exercise programs and weight-loss drugs, with dates
- Your plan's own GLP-1 criteria (ask member services to send them)

If you get a denial, the notice must explain your right to a state fair hearing. In California, members already on a GLP-1 can sometimes keep getting it while a hearing is pending. Our [Zepbound appeal letter guide](/blog/zepbound-appeal-letter/) shows how to answer a denial reason point by point, and the same method works for Wegovy.

## Lost Medicaid coverage? Your options

Talk to your prescriber before you change anything. Stopping a GLP-1 often brings weight back, and only your doctor can help you plan a switch or a pause.

| Option | What it costs (October 2026) | Good to know |
|---|---|---|
| Covered condition (diabetes, heart, sleep apnea) | Nominal Medicaid copay | Needs the right diagnosis and prior authorization |
| Foundayo pill (LillyDirect) | $149 to $299 a month by dose | Self-pay, does not use insurance |
| Wegovy pill (NovoCare) | $149 to $299 a month by dose | Self-pay |
| Zepbound vials or KwikPen (LillyDirect) | From $299 a month | Self-pay |

Our [weight loss pill cost guide](/blog/weight-loss-pill-cost/) compares every self-pay price in detail. Do not switch to compounded, "research-grade" or online peptide versions to save money. FDA has warned about these products, and several were recalled this month (see our [tirzepatide recall guide](/blog/tirzepatide-recall/)).

## Coverage or not, protect your muscle

In the big GLP-1 trials, part of the weight people lost was lean mass, not only fat. That matters even more if your coverage is uncertain, because the muscle you build now helps you keep weight off if you ever have to pause the drug. Two habits do most of the work: enough protein every day and regular strength training. Our [GLP-1 cost and coverage guide](/blog/glp1-cost-and-coverage/) covers private insurance, and our [Foundayo guide](/blog/foundayo-orforglipron-guide/) explains the newest pill option.

[Sharpy](https://apps.apple.com/us/app/glp-diet-app-sharpy/id6762084138) keeps it simple: one protein target, one 20-minute workout and one Shape Score a day, so you can see fat coming off while muscle stays.

## The bottom line

Medicaid covers GLP-1s for diabetes, heart risk and sleep apnea in every state. Weight-loss-only coverage is optional, and 2026 brought cuts in California, Pennsylvania, New Hampshire, South Carolina and Massachusetts. The BALANCE model gives states a cheaper way back in, and Indiana is the first to say yes. Call your plan, ask your prescriber about covered conditions, and keep building strength. [Sharpy](https://apps.apple.com/us/app/glp-diet-app-sharpy/id6762084138) can help with the daily part.

This guide is general information, not medical or legal advice. Coverage rules vary by state and plan.

## Frequently asked questions

**Q: Does Medicaid cover GLP-1 for weight loss?**

A: Only in some states. Covering GLP-1s for weight loss alone is optional for state Medicaid programs. KFF counted 13 states that covered them for obesity in January 2026, down from 16 in late 2025. Call the member services number on your Medicaid card and ask whether your plan covers Wegovy, Zepbound or Foundayo for weight management.

**Q: Does Medicaid cover GLP-1 for type 2 diabetes?**

A: Yes. Under the Medicaid Drug Rebate Program, state Medicaid programs must cover nearly all FDA-approved drugs for their approved uses, and that includes GLP-1s like Ozempic, Mounjaro and Rybelsus for type 2 diabetes. Your state can still require prior authorization or prefer one brand over another.

**Q: Does Pennsylvania Medicaid cover Wegovy?**

A: Not for weight loss alone. Pennsylvania Medical Assistance stopped covering GLP-1 drugs for overweight and obesity for dates of service on and after January 1, 2026, per the Pennsylvania Bulletin notice of December 27, 2025. Coverage continues for other medically accepted uses, with prior authorization. Members under 21 can still be reviewed under the federal EPSDT child health benefit.

**Q: Is Indiana Medicaid covering GLP-1s for weight loss?**

A: Indiana announced on July 30, 2026 that its Medicaid program will join the federal BALANCE model to cover select GLP-1 drugs for eligible members with obesity. The state said eligibility rules and the start date would be announced later, so check with Indiana Medicaid or your plan for the current status.

**Q: What is the BALANCE model for Medicaid?**

A: BALANCE is a voluntary CMS program that negotiates GLP-1 prices with drug makers on behalf of state Medicaid programs. States can join from May 2026 through January 1, 2027. It covers Wegovy, Ozempic, Rybelsus, Mounjaro, Foundayo and the Zepbound KwikPen. Members must be 18 or older and meet clinical criteria, and a free lifestyle support program comes with the drugs.

**Q: How much does a GLP-1 cost with Medicaid?**

A: When Medicaid covers the drug, your cost is limited to a small, nominal copay under federal law, and many states charge nothing. If your state does not cover it, you would pay the self-pay price, such as $149 a month for the starting dose of Foundayo or the Wegovy pill as of October 2026.

**Q: What can I do if Medicaid stopped covering my GLP-1?**

A: Ask your prescriber whether you have a covered condition, like type 2 diabetes, heart disease or sleep apnea. If you get a denial, you have the right to a state fair hearing. Ask about manufacturer self-pay programs. Do not stop your medicine suddenly, change your dose or switch to compounded or research-grade drugs on your own.
